Who we accept referrals from
Hospital Discharge
Fast-track packages for patients leaving Addenbrooke's and other Cambridge hospitals. We work with discharge coordinators to ensure safe transitions home.

Approved for Cambridgeshire County Council and ICB packages. We accept both council-funded and self-funded clients for domiciliary and supported living care.

GP practices and community nursing teams can refer patients who need support at home to maintain independence and avoid hospital admission.

Social workers and care managers can refer adults with learning disabilities, autism and mental health needs for our supported living service.

Dementia, end of life, acquired brain injury, dual diagnosis — we have trained staff for complex packages and work closely with MDT teams.
